If you are in grades 9 to 12 and are planning on playing sports during the 2017/2018 School Year, you must have a current physical on file with the school. Physicals are only good for one year from the date you received your exam. Please stop by Beaver Dam Jr/Sr High School front office Monday - Thursday 7:30 am until 3:00 pm to pick up an Athletic Packet or you can print one from our website.
